From July 27th to 29th, 2018, Time Specialist was invited to participate in the 3rd Accelerated Rehabilitation Surgery and Medical Innovation Summit. Times Specialty Medicine and Marketing Department participated in the study of this forum. Next, share the essence of nutrition and ERAS related content learned in the forum.

What is accelerated rehabilitation surgery?

Accelerated Rehabilitation Surgery (ERAS) is patient-centered and based on evidence-based medicine, with the participation of many disciplines such as surgery, anesthesia, nursing, nutrition, physiotherapy, etc., to complete the optimization of many processes before, during and after surgery; it can slow down Stress, reduced complications, reduced treatment costs, shorter hospital stays, and improved patient satisfaction.

Relationship between nutrition support and ERAS

ERAS is a new management concept in the perioperative period. Nutrition support plays a pivotal role in the ERAS system and is the foundation of ERAS 'success. Malnutrition is one of the important factors affecting the success of ERAS, and the two are inseparable. Only good nutrition can withstand the blow of surgery. Therefore, nutrition and ERAS are inseparable, and they are strategic partners. Nutrition is also an important guarantee for ERAS to achieve rapid recovery. Nutrition support therapy has become an indispensable and important part of integrated medicine, and it has played a very important role in the clinical treatment of various specialties, and directly affects the outcome of patients' diseases. Data show that the major causes of postoperative complications are not only technical but also malnutrition.

The importance of nutritional support

Current evidence shows that reasonable nutritional support during the perioperative period can help reduce decomposition and loss of lean tissue, help patients get out of bed early and recover as quickly as possible, reduce complications, shorten hospital stays, and stay in ICU, thereby improving clinical outcomes. Once there is malnutrition, it will lead to an increase in the risk of surgery, an increase in the incidence of complications, an increase in mortality, an increase in postoperative recovery time, an increase in hospital stay, and a decrease in chemotherapy tolerance. Some research data show that the incidence of postoperative complications of malnutrition is> 20 times normal. Therefore, nutritional support is necessary.

The importance of pre-operative rehabilitation

Nutritional support is necessary and should be considered before surgery. Because preoperative nutritional support can improve the overall state of the patient's body, improve the ability to withstand surgery, and recover from trauma as soon as possible. Many studies and guidelines recommend that nutritional support 7-10 days before surgery can significantly benefit patients with severe malnutrition. The European Association of Parenteral and Enteral Nutrition recommends that patients with moderate to severe malnutrition should be provided with nutritional support before surgery for 7-14 days to delay the operation. Because immediate surgery is not the best option for malnourished patients, preoperative nutritional support is necessary. Especially among pre-operative malnourished patients, many elderly patients and cancer patients have anemia and hypoproteinemia. At this time, if high-quality protein can be supplemented, the body can better promote protein absorption and synthesis and help improve Anemia, then the efficiency of rehabilitation will be greatly improved, and it will also help reduce the number of hospital stays and costs. For example, the concentrated whey protein, soy protein isolate, and collagen peptides contained in Tezhentai in special medical foods are suitable for pre-rehabilitation use. It can supplement the body with high-quality protein and efficiently absorb it, while helping to improve Preoperative nutritional status, enhance immunity and store enough vitality for surgery.
